Plan Fit & Functional Understanding
Does the current plan still reflect the person's needs, communication profile, quality-of-life goals, and the likely functions of behaviour—or have those needs changed over time?
Daily Delivery & Consistency
How consistently is the plan being implemented in everyday practice? Can staff deliver agreed strategies reliably across routines, transitions, and periods of increased demand?
Restriction Reduction Pathway
Are current levels of restriction and staffing proportionate to the person's needs and risks, and is there a clear, evidence-informed pathway to reduce restrictive practices safely over time?
Practice Leadership, Supervision & System Support
Are staff receiving the coaching, supervision, and operational support needed to deliver the plan consistently, or are systemic barriers creating implementation drift?
