It helps identify whether barriers to progress sit within the plan itself, changes in the person's needs, the consistency of day-to-day implementation, or the wider systems supporting delivery.

A PBS Plan & Implementation Review is appropriate when there is already a Positive Behaviour Support plan in place, but there are questions about whether it still reflects the person's needs or whether it is being implemented consistently in everyday practice. Where appropriate, yes. Direct observation may form part of the review to understand how the Positive Behaviour Support plan is being implemented in everyday practice. Observation helps us explore how staff apply agreed strategies, how escalation is recognised and responded to, how consistently support is delivered across teams and shifts, and whether day-to-day practice reflects the written plan. The scope of any observation and document review is agreed during the scoping conversation and is proportionate to the complexity of the situation and the questions the review is intended to answer.

You will receive a structured written report including: Clear findings explaining why progress has stalled Identification of whether barriers sit within the plan, its implementation, the environment, or the wider support system Practical, prioritised recommendations to improve implementation and outcomes A clear implementation plan with agreed priorities for action Guidance on whether the existing plan can be strengthened or whether further assessment or revision is required The report is designed to support proportionate, evidence-informed and defensible decision-making by providers, commissioners and multidisciplinary teams.
