Complex Needs Assessment

A comprehensive Functional Behaviour Assessment (FBA) that goes beyond behaviour to understand the wider factors influencing support, quality of life and risk.

Behaviour, distress and escalating support needs rarely arise from a single cause. Our Complex Needs Assessment combines Functional Behaviour Assessment (FBA), biopsychosocial formulation and real-world observation to understand what is happening, why it is happening, and what needs to change.

Outputs: Functional Behaviour Assessment (FBA), integrated biopsychosocial formulation, identification of key drivers and protective factors, proactive Positive Behaviour Support recommendations, and a prioritised implementation plan to improve consistency, quality of life and reduce avoidable risk.

PBS Plan & Implementation Reviews

Clarify whether the support plan, its implementation, or the wider system needs to change.

When outcomes are not improving, the question is not always whether the plan is wrong. We review whether challenges reflect changing support needs, implementation drift, or barriers within everyday practice, supervision, leadership or the wider service.


Outputs: PBS plan quality review, implementation fidelity findings, identification of barriers to consistent delivery, and practical recommendations to strengthen implementation and improve outcomes.

Capable Environments & System Readiness Reviews

Identify what in the environment, culture and wider system is enabling—or limiting—good support.

Good support depends on more than individual staff practice. We review the environmental, organisational and leadership conditions that influence consistency, quality of life and risk, including routines, capability, supervision, governance and decision-making.


Outputs: System readiness findings, environmental and organisational recommendations, implementation priorities, and practical actions that strengthen everyday support and reduce avoidable restriction.

Specialist Reviews

Used when a specific question needs answering, or where barriers to progress sit beyond the person or the support plan.

Sometimes organisations do not need a full assessment. They need a focused review that answers one important question—why progress has stalled, why implementation is drifting, or what within the wider system is preventing good support.

These targeted reviews are time-limited, evidence-based, and designed to produce clear findings and practical recommendations that help services move forward with confidence.

ENVIRONMENT & CULTURE REVIEW Use when: Day-to-day practice feels inconsistent, overly restrictive, or disconnected from what support plans intend to achieve. What we review: Daily routines, environmental conditions, interaction patterns, predictability, team culture, and the informal practices that shape support under pressure. Outputs: Clear observations, identified strengths and risks, and practical recommendations that improve consistency, reduce pressure, and create the conditions for good support.

LEADERSHIP & SYSTEM READINESS REVIEW Use when: Progress has stalled, restrictive practices remain, or previous training and improvement work has not translated into consistent everyday practice. What we review: Leadership, governance, supervision, quality assurance, decision-making, workforce capability, and how organisational systems support—or undermine—good practice. Outputs: System-level findings, implementation priorities, and practical recommendations that strengthen leadership, improve consistency, and support sustainable change.

TARGETED IMPLEMENTATION AUDIT Use when: The approach is agreed, but outcomes are not improving and you need to understand where implementation is drifting. What we review: Fidelity to agreed practice, role clarity, supervision, coaching, feedback loops, and consistency at the moments that matter most. Outputs: A focused analysis of implementation drift, the reasons it is occurring, and practical recommendations to restore reliable, consistent delivery.

How it works

Clarify the question

Every review begins with a short scoping conversation to understand the decision that needs to be supported, the outcomes you're seeking, and the information required to answer it with confidence.

Understand what is happening in practice

We review the available information and, where appropriate, gather additional evidence through observation, conversations, and document review. The focus is understanding what is happening in everyday practice—not simply what is written in policies or plans.

Produce clear findings and practical recommendations

You'll receive a structured report that identifies what is working, what is contributing to risk or inconsistency, and the practical actions most likely to improve outcomes.

Optional implementation support

Where further support is helpful, findings can be translated into an implementation plan, leadership coaching, or targeted training to help recommendations become everyday practice.
