We undertake functional, formulation-led reviews to understand what is driving distress and behaviour in real-world conditions. This includes reviewing unmet needs, patterns of escalation, environment fit, and the consistency of daily support.
Outputs: clear functional hypotheses, key drivers and protective factors, optimised proactive support plan, and priority recommendations (including where restrictive practice reduction is safely possible).

PBS Plan & Implementation Reviews
Clarify whether the plan needs updating — or the system needs guidance on correct implementation.
When a plan exists but outcomes are not improving, we review whether challenges reflect a change in support needs, or implementation drift (capability, routines, supervision, leadership, or consistency).
Outputs: plan quality review, “fit vs fidelity” findings, barriers to implementation, and practical recommendations to restore consistent delivery.

Capable Environments & System Readiness Reviews
Identify what in the environment, culture, and governance is enabling — or blocking — progress.
We review the universal conditions that make good support more likely: predictability, capability, routines, sensory and environmental factors, and the decision pathways that shape how risk is managed.
Outputs: universal supports (practical everyday changes), system-level drivers of restriction, readiness findings, and priority actions.

Specialist reviews and targeted audits
Used where a specific question needs answering, or where barriers to progress sit beyond the person or the plan.
These reviews can be commissioned on their own, or alongside our main assessments when a more focused or system-level question needs answering. They are time-limited and diagnostic, designed to clarify where barriers are sitting and what needs to change.
ENVIRONMENT & CULTURE REVIEW Use When: Day-to-day practice feels stuck, inconsistent, or overly restrictive, and there is a mismatch between written plans and lived experience. What We Look At: Daily routines, interaction patterns, predictability, pace, and the informal norms that shape how staff respond under pressure. Outputs: Clear observations, strengths and risks, and practical changes to routines and environments that support consistency, reduce pressure, and enable safer, less restrictive support.
LEADERSHIP & SYSTEM READINESS REVIEW Use When: Progress has stalled, reduction pathways are unclear, or restrictive practices persist despite plans, training, or previous reviews. What We Look At: Governance and decision-making, supervision and practice leadership, training and capability systems, quality assurance, and alignment across HR, operations, safeguarding, and leadership roles. Outputs: Identified system-level barriers, key decision points, and prioritised actions to embed the approach consistently and enable sustained improvement.
TARGETED IMPLEMENTATION AUDIT Use When: The approach is agreed and understood, but outcomes are not improving — and you need to pinpoint exactly where delivery is breaking down. What We Look At: Fidelity to agreed practice, role clarity, supervision and feedback loops, and consistency at the moments that matter most. Outputs: A focused analysis of implementation drift and clear, practical recommendations to restore reliable delivery without reopening the whole system.
How it works
Clarify the question and the decision
We start with a short scoping conversation to understand what decision needs to be supported (for example: placement stability, proportionality of support, reduction pathways, or readiness for inspection).
Gather evidence in real-world conditions
We review the key information already available and, where needed, include direct observation and conversations with the people closest to day-to-day delivery. The aim is to understand what is happening in practice, not just what is written on paper.
Provide clear findings and practical next steps
You receive a short, structured set of outputs: what is working, what is creating risk or restriction, and prioritised recommendations that clarify what needs to change — and where.
Optional: turn findings into an action plan
If you want support to embed recommendations, we can provide a focused feedback session, action planning, and implementation support. This is optional and commissioned separately.
